Poe Hat installation instructions

Hi,
I bought some pieces of the poe hat offered through amazon for rpi4b, the
isolated version. I fail to find instructions to mount it, or even a simple picture
to see how it will look like in the end. Can you please give some guidance?

Attach black wire from fan to GND pin on PoE HAT. Attach red wire from fan to 5V (performance) or 3.3V (quiet) pin on PoE HAT. 

5V and ground is required. Most POE HAT connect to the entire GPIO header... Looking for a pic if I can find one

The pins you're looking for on the rpi are the left-hand side pins... here is the header config
