Pi POE Hat v2 Overheating

I was really liking these the two POE Pi hats until recently they started to overheat and make my Pi 4 have several errors that I thought was my memory stick.  I have small fans on a small pi rack server that has two pi's running.  Anyone have experience or know of something I can do?

Picture of the setup? The PoE HATs should be actively cooled. What's your ambient temp and how much airflow do you have going to it?

The Pi on top is the one that I removed the hat and the bottom one still has the hat.  It keeps shutting down.  I have not captured temp data.

Any thoughts from the picture?  I think there is ample air flow even without the fans.  However, I use the fans when the hat is on, but that does not seem to be enough air movement.

@dwhitmire Your fan setup has acrylic both beneath and above it. You need at least 1cm of clearance on the air-in side and about 0.5cm of clearance on the air-out side in order for fans to be effective. The setup you have completely negates the point of having a fan because the fan is not pushing air.
